Here’s a list of answers to the questions we’re most often asked. This is a great place to start, if you’re new to Community Care Partners and wondering where to begin.
Where do I start if I want services for my loved one?
Just give Community Care Partners a call at 732-705-3253 to set up a free, no-obligation consultation. We’ll discuss your needs, walk you through the process, and help you plan next steps.
How do I know if my loved one is eligible for your services?
Eligibility requires three things: (1) a DDD determination of intellectual/developmental disability under N.J.A.C. 10:46; (2) financial eligibility for Medicaid/CCP; and (3) clinical need meeting ICF/ID Level of Care, plus either reaching the top of the CCP waiting list or being declared an emergency. Your support coordinator can verify all criteria.
Who pays for Community Care Partners ID/DD services?
All core services are delivered through New Jersey’s Community Care Program (CCP), a Medicaid waiver funded equally by federal and NJ state Medicaid dollars. There’s no private bill or separate fee to families for covered services.
Is there any out-of-pocket cost for us?
No, there’s no out-of-pocket cost to you for our services. Once enrolled in CCP—and actively managed by a support coordinator—covered services carry zero out-of-pocket expense. Housing costs like rent, utilities, and food remain the family’s responsibility, but we will be happy to help connect you to resources for financial assistance in those areas, if needed.
What happens at the initial consultation?
Our staffing coordinator will review your loved one’s goals and preferences, explain CCP rules, and set up a “meet & greet” with a Direct Support Professional. You interview and choose the DSP who best fits your family’s needs. Services begin once both sides agree.
What kinds of support do you offer?
We provide person-centered, community-based services—daily living skills, employment coaching, medical appointment support, respite, equine-assisted therapy, computer programming, recreational outings, and more—tailored to each individual’s ISP.
How long does it take to get started?
Your turnaround time depends on a few factors. They include DDD eligibility, Medicaid enrollment, and CCP waiting-list status. In emergency situations, DDD’s Intensive Case Management Unit can fast-track access. Your support coordinator will guide timing and keep you informed.
Can I choose or change my Direct Support Professional?
Absolutely. A person-centered meet & greet lets you select a DSP. If it doesn’t feel like the right fit, you can request a new match at any time—no hassles and no red tape.
Do you offer 24/7 support or after-hours help?
Yes, we do! Community Care Partners maintains on-call staff around the clock. A live team member is always available to address emergencies or urgent questions at 732-705-3253. In the event of a medical emergency, you should always call 911 first.
How do I discontinue or modify services?
You’re free to pause, change, or end services at any point. Just notify your support coordinator—there are no cancellation fees or penalties. We’ll work with you to adjust support levels as needs evolve.
