As a New Jersey DDD (Division of Developmental Disabilities) statewide provider Community Care Agency, we are committed to delivering superior Community Care services as well as community support services to individuals with special needs.

We offer a wide range of professional services with experienced and diversified health and community-related disciplines including but not limited to direct support professionals, certified home health aides, nurses, social workers, teachers, and personal trainers, to name a few.

Who We Are

To maintain our high standards, we are approved by the State of New Jersey Division of Developmental Disabilities. Every employee goes through a rigorous process of onboarding: 

​

  • Federal and New Jersey State screening processes

  • Fingerprinting

  • Drug Screening

  • An intensive interview process that includes reference verification and skill evaluation.​

  • Online specific training through Elizabeth Bogg Center at Rutgers University

  • CPR and First Aid Certification 

  • Central Registry from Offenders of Verification

 

Furthermore, our staff receives ongoing specialized training to maintain their skills and is updated on the latest changes in an on-going NJ DDD training. We follow and maintain all state and federal regulations.

Our Standards

ID/DD Services

Intellectual/ Developmental Disability Services

We work closely with Support Coordinators, the families, and participants to ensure that each Individual Service Plan (ISP) is inclusive of the services that are necessary for each individual to succeed at his/her outcomes.

​

We recruit, train, and qualify each of our Direct Support Professionals to provide every individual with the best possible service to achieve goals, fulfill dreams and enjoy new experiences. Because of the importance of connecting the right people together; we set up a Meet and Greet so that each individual has a chance at interviewing and choosing the DSP that is the best fit. This is the key to building long-lasting relationships and encouraging a person-centered approach.

We closely monitor our services by keeping constant and open communication between all team members, as well as conducting ongoing supervisory visits.

Employment Supports

Whether you’re just getting started or have had a job before, Supported Employment services can assist. Employment Job Coaches come from diverse backgrounds. Our job coaches are personable, solid candidates with proven track records of success. 

 

Combining years of both life experience, along with personal achievement, our job coaches and developers are able to identify what consumers are passionate about. Once identified, we carefully select opportunities that are suited for their talents and geographic location.

​

Job coaches, trained through the Elizabeth Bogg Center at Rutgers University, have to date placed individuals in a myriad of positions:
Graphic designers, Computer Programmers, Supervisory Personnel, Sales, Inventory, Warehouse Delivery, Medical Technicians, Dispatchers, Bakers, Route Drivers, and Chefs just to name a few.

WHY CHOOSE COMMUNITY CARE PARTNERS LLC?

There are many agencies to choose from in New Jersey alone, but we are a privately-owned agency. We work one-on-one basis with each client or family that calls us or comes through our door. We take our job very seriously and work hard to achieve excellent results so that our customers are 100% satisfied!

HOW WILL I KNOW IF I WILL LIKE MY DIRECT SUPPORT PROFESSIONAL?

​

Our coordinating team will first ascertain your needs and language preferences and try to find the best support for you. A meet and greet will be scheduled between you and your potential DSP, giving you the opportunity to directly interview him/her. The choice rests with you; we are all about person centered approach! 

​

​

ARE YOUR SERVICES COVERED BY INSURANCE?

​

We accept all NJ Medicaid HMOs, most Private Insurances, NJ DDD Program, Long Term Care, Medicaid, Medicaid Waiver programs, Veterans assistance, and Private Pay.

HOW DO YOUR SERVICES WORK?

​

After an initial consultation with our Staffing Coordinator, we will schedule a Meet and Greet with a Direct Support Professional. If both parties agree to work with each other, service can begin. If at any time you wish to discontinue our services, you are free to do so with no penalties or obligations; you can cancel at any time.

CAN I REACH SOMEONE AFTER HOURS?

​

Yes! We have on-call staff 24/7. We always have a live person available to help with any emergencies after hours.

WHO IS RESPONSIBLE FOR ALL THE SOCIAL SECURITY, FEDERAL, AND STATE TAXES OF THE CAREGIVERS?

​

Since all of our caregivers are employed by Community Care Partner LLC, the company is fully responsible for their social security, Medicare, disability, and all state income tax withholdings. We make sure that our clients and their families are not responsible for these tax liabilities. If they hired someone independently or through a staffing agency, the family would be responsible. Please call us for more information!
