If you are new to I/DD services, we make the process simple. Reach out to Community Care Partners and we will guide you through the DDD system, help you find a Support Coordinator, and work with your family to build a service plan that truly supports your loved one’s goals.

How We Can Help

Our Support Process

We make it simple to start I/DD services in New Jersey. Our caring team guides you through the DDD process, helps you understand your options—such as funding through the New Jersey Community Care Program for individuals with I/DD, and works with your Support Coordinator to build a plan that fits your loved one’s needs

Family Hire Program

Many families want care from someone their loved one already knows and trusts. Our Family Hire Program makes that possible. We help you hire and train a relative or close friend to become a Direct Support Professional (DSP).

This gives your loved one consistent support from someone who understands their routines, strengths, and challenges. We guide you through every step so the process feels clear and manageable.

What We Help With

  • Completing DDD paperwork and requirements
  • Training and onboarding your chosen caregiver
  • Building a service plan that supports daily needs
  • Ongoing guidance for your family
