Who We Are

We’re a group of compassionate, trained and licensed professionals who find purpose and meaning in supporting people with Intellectual and Developmental Disabilities (I/DD) and their loved ones. Many of us also have loved ones with I/DD ourselves, making this work deeply personal to us.

What We Do

People with intellectual and developmental disabilities often encounter unique hurdles in today’s world. Community Care Partners offers practical services and hands-on support to help your loved one thrive. Our commitment is to enrich the daily experiences and well-being of every person we serve.

Some of the support services we offer:

  • Assistance in finding appropriate ID/DD housing.
  • Job training and assistance in resume writing and finding work.
  • Scheduling medical appointments & transportation to/from them. 
  • Teaching people with ID/DD self-advocacy and self-care
  • Self-care and living skills coaching for people with disabilities. 
  • Connections to exercise and recreation opportunities.

How We Do It

​We partner with Support Coordinators, families, and participants to build Individual Service Plans that include everything someone needs to reach their goals.

Our Direct Support Professionals are carefully recruited, trained, and qualified to help each person chase dreams, try new experiences, and thrive. We even host Meet & Greets so everyone can meet—and choose—the DSP who feels like the best fit

To keep our services on track, we maintain open communication across the team and conduct regular supervisory visits, ensuring everyone stays aligned and supported.

Community Care Partners is a New Jersey State Division of Developmental Disabilities Approved Provider. Every employee goes through a thorough vetting and screening process before they can begin onboarding training: 

Our rigorous screening and onboarding process includes:

  • Fingerprinting
  • Drug Screening
  • CPR and First Aid Certification 
  • Central Registry of  Offenders  Verification
  • ​Federal and New Jersey State screening processes
  • Intensive interview including reference verification and skill evaluation.​
  • I/DD specific training through the Elizabeth Boggs Center at Rutgers University

Community Care Partners staff receive regular specialized training to ensure skills maintenance and are updated on the latest changes in NJ DDD training. We follow and maintain all state and federal regulations.
