ID/DD Support Services for Your Loved One
Community Care Partners delivers useful services for people with developmental and/or intellectual disabilities. Our organization is designed to make it as easy as possible for those closest to people with ID/DD to secure practical support for their loved ones. We provide everything from hands-on services to advice and guidance.
Community Care Partners also acts as a bridge, connecting you to state government and Support Coordinators. We work to ensure that every family is aware of the resources available to support their loved ones with ID/DD and facilitate their access to them.
Accessing I/DD Services for Your Loved Ones

We work closely with Support Coordinators, the families, and participants to ensure that each Individual Service Plan (ISP) is inclusive of the services that are necessary for each individual to succeed at his/her outcomes.

How It Works
We recruit, train, and qualify each of our Direct Support Professionals to provide every participant with the best possible service to achieve goals, fulfill dreams and enjoy new experiences. Because of the importance of connecting the right people together; we set up a Meet and Greet so that each individual has a chance at interviewing and choosing the DSP that is the best fit. This is the key to building long-lasting relationships and encouraging a person-centered approach.
We Also Hire Family to Care for Family
In addition, a family member can serve as the DSP for the participant, ensuring both a safe and familiar care environment while providing peace of mind that the participant is receiving the quality support they deserve. We guide family members through a simple and supportive onboarding process, making it easy for them to get started, while also allowing them to be compensated for their dedication.
We closely monitor our services by keeping constant and open communication between all team members, as well as conducting ongoing supervisory visits.
How it works, in a nutshell:
- We collaborate with Support Coordinators, families, and participants to build ISPs covering every needed service.
- We recruit, train, and match Direct Support Professionals in Meet & Greets so participants pick their best fit or we onboard the Family member that is willing to serve as the DSP
- We keep open communication and conduct regular check-ins to ensure quality support.
